Urinary incontinence or urine leakage means that the patient is not able to retain the urine in the bladder. Due to the loss of control, the urine leaks quite often.   • Medicines – Medications help in relaxing the bladder and regain more control over the urination. Anticholinergics, estrogen creams, antidepressants, and alpha-blockers are prescribed by the urologist for urinary incontinence treatment as per the type of urine leakage. 
  • Behavioral training for Urinary Incontinence Treatment – It is an integral part of urinary incontinence treatment. The patient is taught a few techniques to follow. Double voiding, urinating every 2-3 hours and Bladder training are taught to the patient. The behavioral training helps in increasing the control over the bladder and increases the quality of life of the patient.
  • Pelvic Floor Exercises or Kegels – These exercises are taught to the patient to regain muscle strength in bladder muscles. The patient is taught to squeeze the bladder muscles and hold them for a few seconds. A set of repetitions are advised. They work best in case of stress and urge incontinence. 
  • Botox Injections – These botox injections work best in urge incontinence or in overactive bladder. They are given in a specified quantity to relax the bladder muscles. In some cases, a thickening material is injected inside the urethra to add bulk to it and control the leakage. 
  • Urine Absorbent Paddings – There are urine absorbent paddings that can be worn to manage any urine leaks. These paddings omit the need for a bulky diaper. Drip collectors are given to males suffering from urine leakage that collects the urine. It is worn over the tip of the penis. 
  • Inserts for Women – Some pessary or urethral insert devices can also help to prevent urine leakage. They work in females only as they need to be inserted inside the urethra to prevent leaks before going out. They need to be removed before urinating. 

Urinary Incontinence Treatment by Surgery 

  •  The Mesh sling procedure is one of the best urinary incontinence treatments in Delhi, India done by Dr. Vineet Malhotra. In the surgery, a mesh is used to create a sling for the urethra. This sling supports the urethra and keeps it closed. This prevents urine leakage. It is inserted with a minimally invasive procedure known as laparoscopy. 
  • Urethral sphincter Surgery – A ring-like sphincter is implanted in the urethra. This ring remains inflated all the time and it keeps the urethra closed. When the patient wants to urinate, the ring is deflated by pressing a button on the skin. This allows the patients to urinate whenever required without any leak episodes in between.
  • Bladder neck suspension. In this surgery, the bladder neck is suspended by giving a few sutures in the vaginal wall. It is performed by laparoscopy technique to provide support to the bladder neck. 

Dietary Changes For Urinary Incontinence Treatment

Bladder irritants like coffee, carbonated drinks, and alcohol-based beverages should be avoided. Healthy meals with high fiber content are suggested to ensure regular bowel movements. If the patient is obese then dietary modification is advised for that as well.

Complications of Urinary Incontinence

  • Skin infections and urinary tract infections can occur due to continuous leakage of urine. It can cause foul odor and rashes etc. 
  • The patient remains confined at the home and does not want to attend social gatherings due to the embarrassing nature of urinary incontinence.

Types of Urinary incontinence

  • Stress Incontinence – Any stress on the abdominal muscles such as lifting heavyweight, coughing, laughing and sneezing, etc can result in the leakage of urine.
  • Urge Incontinence – There is a sudden urge to urinate and this urge is quite frequent. The patient feels the need to urinate even if the bladder is not completely full.
  • Overflow incontinence – In overflow incontinence, the urine continuously dribbles or leaks as the bladder can not empty fully. 
  • Functional incontinence – This type of incontinence is due to any kind of functional issue such as arthritis in the knees that makes it difficult for the patient to reach the toilet. Also, Arthritis in the fingers makes the patient unable to remove the clothes in time, etc. 
  • Mixed urinary incontinence – There is a mix of two types of urinary incontinence in a patient. This makes it the most severe kind of urinary system incontinence. The urine keeps on leaking and the patient needs to wear a diaper most of the time.
